Understanding Keystone Ridge
Small enough

Keystone Ridge is a three-bed, staff-secure Level III residential treatment facility in Winston-Salem for children and adolescents ages 12–17. Young people live here with 24-hour staff supervision and support.

Each resident has an individualized assessment and a Person-Centered Plan. Staff implement and document that plan daily.
- The everyday matters
Support includes meals, school attendance, hygiene, appointments, transportation and community activities according to the resident’s plan.
- Connected, not isolated
The program coordinates with family or guardians and the wider care team, including schools, clinicians and care managers.

Does a young person need to be in DSS custody?
No. Many placements involve county DSS custody or guardianship, but DSS custody is not a universal requirement. Admissions reviews legal authority alongside clinical information, supervision needs and the authorization path.

How long does initial review take?
Initial review is typically completed within 2–4 weeks of receiving a complete referral. Placement also depends on fit, authorization and admission readiness. A waiting list is maintained when the facility is full.
What makes a placement final?
Written payer authorization, a signed Service Order and admission paperwork must be completed before placement is final. Sending documents starts or supports a review; it is not confirmation of admission.
Who can contact the team?
Families, legal guardians, DSS social workers, care managers and clinicians can inquire. Keystone serves referrals from across North Carolina.
