Start with the young person's needs
A residential placement is a decision about the support a young person needs, not simply a search for an available room. Families and referring professionals need to understand the setting, the clinical recommendation and the practical responsibilities that come with a move. Those conversations are easier when everyone distinguishes a referral for review from an approved, ready-to-begin placement.
Keystone Ridge, operated by Keystone Care Network, LLC, is a three-bed, staff-secure, Level III residential treatment facility in Winston-Salem, North Carolina. It serves children and adolescents ages 12–17. Residents live in the program with round-the-clock staff supervision and support. The licensed service category is Residential Treatment, Staff Secure, for Children or Adolescents. The level of care should be considered alongside the individual's assessment, not used as a substitute for it.
Understand what daily support includes
Residential care brings treatment planning into everyday life. At Keystone Ridge, that includes routines around school, meals, hygiene, chores, appointments and transportation, together with therapeutic and skills-focused support. Staff implement each resident's Person-Centered Plan, or PCP, and document that work daily. The plan connects the reason for placement with the support offered during ordinary activities.
The program is trauma-informed and person-centered. Behavioural support, de-escalation and crisis response are part of its work. A Licensed Professional provides clinical consultation and oversight, and a Qualified Professional provides on-site qualified-professional services as required under the license. These responsibilities complement daily care; they should not be confused with a promise that every type of clinical service is delivered on site.
A Level III referral starts a fit review. Admission also requires current clinical recommendations, a signed Service Order, written authorization and completed admission paperwork.
Expect a review of more than age
Being within the age range does not by itself establish eligibility or fit. Review considers diagnosis and the current clinical picture, supervision needs, current risk information, legal or custody authority, school needs, the payer authorization path, and bed and staffing fit. An accurate description of present needs helps the team assess whether this setting can provide the support being requested.
Many placements involve county DSS custody or guardianship, but DSS custody is not a universal requirement. Families, legal guardians, social workers, care managers and clinicians may be involved in a referral. Young people from Forsyth County and other North Carolina counties have been referred and served. Identify the people responsible for clinical planning, consent and funding early, rather than assuming a single referrer controls every decision.
- Describe current supervision and support needs, including what has changed since the last assessment.
- Identify the legal guardian or other responsible authority and the relevant care-team contacts.
- Bring school needs and existing supports into the conversation before a move is arranged.

Make the next conversation specific
Initial review is typically completed within 2–4 weeks of receiving a complete referral. That is a review timeframe, not a promised move-in date. Tours can be arranged for guardians and referring agencies, and a waiting list is maintained when the facility is full. Ask which documents are missing and which decisions remain open before arranging a transition.
