Bring education into the first review

A residential move involves more than arranging where a young person will live. School records, existing supports, enrollment responsibilities and daily attendance all need attention. Leaving those conversations until every clinical document is complete can leave the team with unanswered questions at an important transition. School needs belong in the placement discussion from the beginning.

Keystone Ridge is located in Winston-Salem, where the local school district is Winston-Salem/Forsyth County Schools, or WS/FCS. School needs are part of Keystone's eligibility and fit review, and daily-living support includes school attendance and transportation according to each resident's plan. A move should not be treated as proof that enrollment arrangements or educational decisions have already been settled.

Gather records that explain the current picture

Step 2 of the admission portal requests Current School Records and IEP/504 Documentation if applicable. An Individualized Education Program, or IEP, and a 504 plan describe educational supports where those plans apply. Keep their conditional status clear: not every child has one, and the absence of a plan should not prompt someone to invent a replacement document.

Ask the current school or the responsible team member for the records needed to describe the student's present situation. Identify who can clarify existing supports and answer questions about the educational transition. A useful handoff distinguishes records already received from those requested but still outstanding, so the next person does not mistake a request for a completed transfer.

Use the additional DSS-custody checklist in order

Step 3 applies to enrolling a child who is in DSS custody in WS/FCS. It names four items in a fixed order. These are additional enrollment documents, not replacements for the school records and applicable IEP or 504 documentation requested in Step 2. Maintaining that distinction helps the referring team see both what is needed for admissions review and what is needed for the school handoff.

The three DSS forms are provided by DSS. Work with the responsible social worker to obtain the appropriate documents rather than creating a form from a title or copying an unrelated version. The checklist below preserves the document names used in the portal. Confirm questions about completion with DSS and the receiving school contacts.

Clarify authority and practical responsibilities

Many Keystone placements involve DSS custody or guardianship, but DSS custody is not a universal admission requirement. Do not apply the DSS-specific enrollment list to every family without considering the child's legal circumstances. Identify the legal guardian or responsible authority and ask which school enrollment steps apply. A clinical recommendation for placement does not, by itself, explain who may make educational decisions.

The team may include the guardian, DSS social worker, school staff, care manager, clinicians and residential staff. Naming each participant is not enough; identify who is obtaining records, who is communicating with the school and who will confirm the next practical step. This avoids a handoff in which everyone is informed but nobody owns the unfinished task.

  • Who is requesting and sending current school records?
  • Who can explain existing IEP or 504 supports, if applicable?
  • Who will confirm the enrollment requirements and required signatures?
  • Who will coordinate attendance and transportation arrangements with the residential team?

Connect paperwork to the daily routine

Once the responsible parties are working through enrollment, discuss the practical school-day routine alongside the resident's plan. Ask what information staff need to support getting ready, attending school and communicating concerns. Bring unresolved questions back to the appropriate school or care-team contact instead of assuming residential staff can independently decide every educational matter.

Keep the young person's perspective in the conversation. They may have questions about the change that are not answered by a completed form. Make room for those questions without promising a particular school assignment, service arrangement or outcome. Explain which decisions are confirmed and which still require coordination. That distinction is more useful than reassurance that outruns the available information.

Submit updates without losing the larger picture

Keystone's admission portal permits multiple uploads throughout the admission process. Send available records with clear identification, then provide updates as the responsible parties complete them. School documentation is one part of admission readiness; current clinical recommendations, a signed Service Order, written payer authorization and other required admission paperwork remain necessary.
